1.Pathophysiological Studies of Subtotal Colectomy in Dogs:Motility of the Ileum and its Relation to Diarrhea

In our department, it has been already demonstrated that in dogs, subtotal colectomy with ileoproctostomy (Group 2) has brought about increase in ileal peristaltic movement in an early period within 1 to 4 weeks, and subsequent functional hypertrophy of ileum. In this paper is studied the influence of intraluminal pressure of the residual rectum and peristalsis of the ileum.<BR>Seventy-nine mongrel dogs were used and were operatively devided into 3 groups so that Group 1 consisted of those with subtotal colectomy with abdominal ileostomy; Group 3, Subtotal colectomy with ileoproctostomy and anal Sphincterotomy and Group 4, Subtotal colectomy with ileo-segmental colostomy with colostomy. Postoperatively were recorded the measurments of intraluminal pressure of rectum, the electromyographical change of terminal ileum, the peristaltic movement with X-ray cinematography and measurments of the thickness of muscular layer of terminal ileum, in addition to body weight, serum protein, hematocrit, cross findings of feces and mouth anus transit time.<BR>In these experiments all animals but one of Group 1 died within 2 or 3 weeks. Group 2, Group 3, Group 4 and Group 1 were thus observed in order of improvement of general findings. The mean intraluminal pressure of rectum was 164 mm H<SUB>2</SUB>O in Group 2 and 65 in Group 3 and 4. In Group 3 and 4, peristalsis of ileum was much decreased and the hypertrophy of ileal muscular layer was found not meaningful earlily in the postoperative period.
