In cases of pancolectomy, two major continence factors, reservoir and sphincteric continence, were discussed experimentally and clinically.<BR>On this study, the rectum was denuded in the submucosal layer, preserving whole muscular cuff of the rectum, puborectalis sling and sphincters.<BR>The terminal ileum of 5 cm experimentally, of 10 cm clinically was reversed. And the reversed terminal ileum was pulled down through the rectal muscular cuff and ileo-anal anastomosis were performed. By such procedures, good reservoir continence was maintained and good sphincteric reflex was manifested by double baloon technic.<BR>The dogs kept good fecal continence (reservoir, sphincteric continence) and the patient with chronic ulcerative colitis was able to defecate voluntarilly, but fecal sensation was somewhat poor.<BR>Clinically, after the mucosal anesthesia, normal recto-sphincteric reflex was remained. In suggested that the importance of preservation of distal 5-10 cm whole rectum to maintain fully normal fecal continence and sensation.<BR>But considering of radical cure and recurrence of the lesions, these procedures should be discussed.
