An autopsied case of the Marfans syndrome was reported with the huge hydropericardium, dissecting aneurysm and atrophy of the endocrine organs. Histologically, the aneurysmal wall showed cystic medial necrosis and the cusps showed myxomatous degeneration as in previous reports and in the pericardial sac, no inflammatory lesions were observed. The author suspected that the cause of the hydropericardium might be the obstruction of the pericardial vein by the aneurysm of the root of the aorta.
