広島市内の医療機関におけるアドレナリン自己注射液の処方に関する調査

Food allergy is caused by dietary allergens through the oral route, and sometimes develops lethal anaphylaxis associated with severe hypptension. There is a report that 22 patients with food allergy died of anaphylaxis through the period from 1995 to 2003 in Japan. Anaphylaxis appears generally within 30 minutes after taking foods responsible for the disease, so that medical treatment with adrenaline is necessary as quickly as possible to prevent the worse of the reaction. Currently self-injectable adrenaline is avalable to patients with food allergy for a medical support item, which is commrcially named Epipen and is prescribed by Epipen regustered doctors in hospital. Therefore the patients carrying self-injectable adrenaline can access immediate medical attention by themselves before hospital care, when they have a sudden attack of severe anaphylaxis accident. In this study using the method of questionnaires against 42 Epipen registered doctors in Hiroshima-city, we surveyed prescription of self-injectable adrenaline for patients with food allergy. 21.2% of the doctors prescribed self-injentable adrenaline for food allergic patients, half of who were noticeably infants. The doctors who prescribed self-injectable adrenaline had a view about the item to be the emergency first aid against anaphylacis caused by food allergy, while the doctors who did not were in fear and anxiety of the misuse accidents of hte item in especially infantile patients apart from their parents, for example in schools and other childcare settings. To dissolve these problems disturbing the sidtribution of self-injectable adrenaline to patients suffering from food allergy, the educational opportunitiy to learn the handle with care of item is primary for not only medical personnel but also school staff.
