[症例報告]びまん浸潤型大腸癌の1例

A 48-year-old man was admitted to the University Hospital for surgery of the colon on October 16, 1985. About one month prior to this admission, the patient was seen in a local medical clinic with a complaint of abdominal pain. Barium enema studies at the clinic demonstrated a complete obstruction of the transverse colon, and then he was diagnosed to have carcinoma of the colon. On admission, the patient appeared not critically ill. Physical examination revealed tenderness in the upper abdomen. Laboratory data and values of tumor markers were whithin normal limits. On October 28, he was operated on. Extensive spreads to the liver, omentum and regional lymph nodes were noted. Right henucolectomy was carried out. Grossly, the involved colon revealed carcinoma of scirrhous type(Borrmann's 4th type). Histologically, carcinoma showed moderately differentiated tubular adenocarcinoma with marked vascular and lymphatic invasion. The patient was treated with adjuvant chemotherapeutic agents(Mitomycin C, 5-fluorouracil), but expired with clinical manifestations of peritonitis carcinomatosa four months after surgery.
