A 38-year-old male was admitted because of gross h e maturia, and routine urological examination failed to discolose the cause. He was then diagnosed as having essential hematuria for which various kinds of treatments described in the literatures were attempted without improvement. Finally, methylpredonisolon (Medrol) was successfully administered eliminating his renal bleeding. In clinical and laboratory examinations, intradermal staphylococcus vaccine reaction was positive and there was prolonged plasma clotting time under the addition of calcium chloride. Therefore, allergic mechanism can be presumed in this disease as one of the etiologic factors. Changes in clotting time of blood plasma containing calcium chloride were studied in immediate allergy sensitized with eggalbumin as well as in delayed allergy sensitized with BCG, however, no definite tendency was recognized in any case. The mechanism of renal hematuria might be interpreted as any of vasodilatation, increased capillary permeability and coagulation capacity being abnormal alone or together.
