出血性ショックに伴う肺不全の成因に関する実験的研究 ―肺循環障害に至る血行動態上の変化について―

The pathogenesis of pulmonary insufficiency in hemorrhagic shock remains incompletely explained. However it is confirmed that pulmonary arterial pressure increases in the shock lung with pulmonary vascular resistance elevated. This circulatory alteration is considered as an important factor to result the shock lung, but it is not clear where and how it develops and proceeds in pulmonary arterial system, capillary bed or venous system. This study aimed at clarifing it, so that the changes in hemodynamics were observed in process of hypovolemia and post-reinfusion. It is revealed that pressure gradients increased remarkably between small pulmonary veins and left atrium early in shock and between pulmonary arterial wedge and small pulmonary veins late in shock and after reinfusion. This suggests early venoconstriction with alveolar capillary blockage developing secondarily, paticularly prominent after reinfusion increases pulmonary congestion and peri-capillary extravasation. In the present study "Micro-catheterization technique" which the author devised was successfully used to obtain accurate pressure measurements in small pulmonary veins retrogradely.
