In order to study the role of serum albumin by lipolysis of fat metabolism, experiments were made clinically and experimentally, and the results obtained were as follows. 1. In the tests with 131(131上付)I-triolein and butter administration on patients with liver cirrhosis, nephrotic syndrome and normal control subjects, a decrease of fat uptake was seen in patients with liver cirrhosis as compared against patients with nephrotic syndrome and normal controls a decrease of clearing activity of exogenously administered fat (butter) was seen in patients with nephrotic syndrome. 2. Significant increases of serum albumin level were seen after butter administration not only in normal control subjects but in patients with liver cirrhosis and with nephrotic syndrome. 3. In animal experiments using cirrhotic and nephrotic rats, a decrease in fat absorption was seen in cirrhotic rats compared with those in normal and nephrotic rats. 4. Increases of serum albumin levels after butter administration were also seen in normal control rats, and also in rats with cirrhosis and nephrotic syndrome. The rats with a serum albumin increase were higher in the latter two rat groups than the normal control. 5. Although approximately 90% of free fatty acid (FFA) were found in serum albumin two hours after administration of butter in normal controls and rats with cirrhosis of the liver, only about 50% of FFA were found in serum albumin in rats with nephrotic syndrome. Thus, it is conjectured that serum albumin plays an important role as a carrier of free fatty acids not only in fasting state but also in lipolysis of fat metabolism.
