In order to investigate the role of serum haptoglobin (Hp) in hemoglobin (Hb) metabolism, comparative studies between serum Hp levels and occurrences of hemosiderosis following large volumes of blood transfusion were performed clinically, and studies on the degradation of 59Fe labelled Hb or 59Fe labelled Hb-Hp complex by the reticuloendothelial tissues were carried out experimentally. The results thus obtained were as follows : 1) Among 47 autopsy cases who had received blood transfusion in doses exceeding 1,000 m? , the hypohaptoglobulinemic group (less than 30 mg/100 m?) showed the significantly higher rate of 55.6% of the frequency of hemosiderosis, compared with that of 10.4% in the normo-haptoglobinemic group. On the other hand, no case of hemosiderosis was observed in hyperhaptoglobinemia. As to the relationship between transfusion volumes and the occurrence of hemosiderosis, no significant difference was indicated between the group which received more than 8,000 m? of transfusion volume and the group with less than 8,000 m?. 2) Degradations of 59Fe labelled Hb or 59Fe labelled Hb-Hp complex by the isolated stellate cell fraction, parenchymal cell fraction of the liver, and by the spleen tissue homogenate were compared against each other, and no difference of acceleration of the degradations among these cell fractions were found. However, it was, in contrast, clearly accelerated after an addition of ascorbic acid in the incubation medium. It is concluded from these results that Hp in serum may well play a role to prevent hemosiderosis of the tissues which might occur due to relatively large volumes of blood transfusion, and that the degradation of Hb-Hp complex did not occur enzymatically, but rather by a non-enzymatical process.
