In order to study the role of α1-antitrypsin (AAT) in patients with lung cancer, measurements of serum AAT levels and tumor extracts, analysis of serum Pi phenotypes, and measurement of 3H-thymidine uptake by PHA-stimulated lymphocyte blastoformation with serum were performed.The results obtained were as follows: 1) Serum AAT levels measured by the single immunodiffusion method were significantly elevated in 98 cases of lung cancer patients with no evidence of inflammatory diseases, with higher levels being observed in advanced lung cancer patients. 2) Serum Pi phenotypes analyzed by acid starch gel electrophoresis were of the normal MM type in all patients. 3) The size of tumors on chest X-ray and serum AAT levels correlated in patients with squamous cell carcinoma. In patients with adenocarcinoma, serum AAT levels did not correlate with the size of tumors. Higher AAT levels were observed in patients with chest X-ray findings showing convergence of bronchi and blood vessels towards the lesion. 4) In 14 curative operated cases, serum AAT levels after the operation significantly decreased. 5) The concentrations of AAT in lung cancer tissue extracts were not higher than those in normal lung tissue. 6) PHA-stimulated lymphocyte blastoformation measured by 3H-thymidine uptake was inhibited by the serum of lung cancer patients. The degree of inhibition and the concentrations of AAT in the serum of the patients were significantly correlated. 7) Purified AAT from sera of lung cancer patients and normal subjects inhibited lymphocyte blastoformation dose-dependently, and this inhibitory effect of the former was greater than that of the latter. These results indicate that there is significant relationship between serum AAT and the progression of lung cancer, that AAT is an inhibitory factor of cell-mediated immunity, and that the measurement of serum AAT levels may be useful as a parameter for estimating the prognosis of patients with lung cancer.
