Pancreatic endocrine function in obesity and aging was investigated from the viewpoint of changes in plasma insulin (IRI) and glucagon (IRG) after 50 g oral glucose administration. The results obtained were as follows : 1) Significant elevations in plasma IRI and IRG during oral glucose load were observed in obese subjects as compared with those in nonobese subjects. IRI/IRG molar ratios also showed a marked elevation in the former. 2) Plasma IRI had a tendency of lower responses and IRG showed higher levels reversely in obese borderline subjects as compared with those in obese normal subjects. In obese diabetic subjects, however, plasma IRI decreased and IRG increased significantly as compared with those in obese normal subjects. IRI/IRG molar ratios indicated lower levels initially and higher levels gradually following glucose loading in obese borderline subjects and markedly low levels in obese diabetic subjects as compared with obese normal subjects. 3) Obese subjects were classified into two groups, namely under 50 years of age (Y) and over 50 years of age (0); IRI, IRG and IRI/IRG molar ratios decreased in the latter group as compared with those in the former. 4) In nonobese subjects, almost no difference was seen between Y and O group in normal and borderline subjects, but IRI and IRG showed higher values in the latter group in diabetic subjects. However IRI and IRG molar ratios were almost the same in Y and O group. In conclusion, the interrelation of plasma insulin and glucagon may play an important role in abnormalities of glucose metabolism in the presence of obesity. In addition, it was conjectured that decreased carbohydrate tolerance accompanying aging might be closely related to the presence of the so-called obese conditions.
