In order to study the mechanisms of aldosterone secretion, plasma aldosterone concentration (PAC), plasma renin activity (PRA), and total plasma corticosteroid (TPC), serum and urinary electrolytes (Na, K) were measured in eighteen patients with essential hypertension (EHT) following long-term (4 weeks) sodium restriction (less than 35 mEq/L). Furthermore, the differences in the changes in these variables were examined between low and normal renin hypertension. The results were as follows. 1. PAC and PRA significantly increased in both supine and upright positions after 1 week sodium restriction and remained almost constant during a 4 week period. Although there was a significantly positive correlation between PAC and PRA during the short-term restriction (before and at 1 week), no correlation was observed thereafter.2. There were significant decreases in PV and in serum and urinary Na/K ratio as wellll as significant correlations between PAC and PV or urinary Na throughout the whole restriction period. 3. There was a significant decrease in serum Na after 2 weeks and a significant increase in serum K from 2 to 4 weeks following the restriction. 4. TPC did not change significantly in the present experiment. 5. ΔPAC, that is the increment of PAC after postural change, did not alter significantly, while ΔPRA rose significantly, showing a dissociation between ΔPAC and ΔPRA. 6. PAC/PRA and ΔPAC/ΔPRA, as an indicator of the adrenal sensitivity to angiotensin II (AT-II), were almost constant in the supine position and decreased in the upright position. 7. In low renin essential hypertension, there were significantly low PRA, and high PAC/PRA or ΔPAC/ΔPRA raios in both supine and upright positions before the restriction, compared with those of normal renin essential hypertension. These differences between the two groups disappeared following long-term sodium restriction. Thus, the renin status in essential hypertension changed under this condition. These data suggested that, in addition to renin-angiotension system, altered PV or electrolytes such as Na and K, might playa an important role in the maintainance of high PAC following long-term sodium restriction.
