Multiple lymphomatous polyposis (MLP) of the gastrointestinal tract is a rare entity, characterized by numerous polyps affecting long segments of the gastrointestinal tract. MLP of the gastrointestinal tract is believed to be an extra nodal counterpart of the nodal mantle cell lymphoma (MCL), because of its histologic and immunophenotypic similarities with MCL. The immunoglobulin VH (Ig-VH) region sequence of MCL have previously been shown to have no or very little somatic mutations. In this report, we analyzed Ig-VH region genes of a MLP case. This case had MLP in a broad area of digestive tract from the esophagus to the rectum. Genotypic study by Southern blotting revealed that each biopsy specimens had the same gene rearrangements of the immunoglobulin heavy chain. In addition, monoclonal VDJ rearrangements were observed by sequencing of Ig-VH region. Several somatic mutations were seen in Ig-VH region sequences. These results suggest that lymphoma cells of MLP are derived not only from mantle cells but also from lymphocytes of other developmental stages.
