Recent studies have unequivocally shown that preconditioning with brief ischemia limits myocardial necrosis and arrhythmias following coronary artery occlusion. However, effects of preconditioning on myocardial stunning remain poorly characterized. The present study aimed to test the hypothesis that preconditioning may attenuate myocardial stunning by activation of adenosine receptors. In an anesthetized open chest rabbit, regional systolic thickening fraction (TF) was measured using an epicardial Doppler sensor. In the first series of experiments, myocardial stunning was induced by 10 min of coronary occlusion; rabbits had been either untreated, preconditioned with 1 min ischemia at 6 min before the 10 min ischemia, given 8-phenyltheophylline (8-PT, 10 mg/kg i. v.) 21 min before the ischemia, or given both 8-PT and 1 min-preconditioning. Postischemic recovery of thickening fraction in preconditioned rabbits was 84.3±5.0% of the baseline value at 30 min after reperfusion, which was significantly higher than the value in the untreated group (51.3±8.1%). This beneficial effect of preconditioning was not detected in the rabbits given 8-PT before preconditioning, and the time-course of TF in 8-PT group was similar to that in the Controls. In the second series of experiments, the effect of 1 min of preconditioning was assessed when myocardial stunning was induced by 5 min ischemia. Thickening fraction at 30 min reperfusion was again higher in the preconditioned heart than untreated hearts (92.2±1.3% vs. 75.9±3.2%, p<0.05). These findings suggest that preconditioning protects the myocardium against stunning via adenosine receptor activation in rabbit hearts.
