Endoscopic ultrasonography (EUS) is a useful imaging technique for the upper gastrointestinal tract because of its high resolution and lesser degree of image artifacts such as ultrasound attenuation, sidelobes, or deterioration of ultrasound beam profiles frequently experienced in transabdominal ultrasound. Diagnostic image interpretation, however, largely depends on subjective judgement of EUS images obtained during manipulation of the instrument. Developments in interpretation methods are particularly desirable for differentiation between benign and malignant submucosal tumors of the stomach, since it is difficult to take biopsies of such tumors by gastroendoscopy. The present report describes an attempy to differentiate submucosal tumors of the stomach based upon the texture analysis of EUS images using a digital image processing protocol. The texture parameters adopted were derived from the first-order gray level distribution and the second-order co-occurrence matrix. These parameters were used to characterize images of the following submucosal tumors of the stomach: 13 cases of leiomyoma, 11 cases of leiomyosarcoma, 6 cases of carcinoid tumors, 3 cases of lipoma, 3 cases of ectopic pancreas, two cases each of schwannoma and eosinophilic granuloma and one case of fibroma. The discriminatory function was calculated for leiomyoma and leiomyosarcoma based upon the distribution of each parameter and the accuracy of image interpretation was estimated assuming a normal distribution. The digital image discrimination method was found to be relatively simple and accurate. Combined texture analysis using the parameters mean gray level, skewness, kurtosis, SNR and entropy proved to be effective; differentiation was further improved to 96% by incorporation of second-order parameters, except for the interia of the second-order matrix, which had no effect. The mean gray level showed a quite close correlation with either high cellular density or high density of fibrous components in the mass lesions. This method appears to have high potential for diagnosis of malignancy not only in myogenic tumors but also in many other submucosal tumors.
