びまん性肺疾患及び原発性肺腺癌患者血清における抗SP-A単クローソ抗体反応物質の測定とその臨床的意義

An immunoassay kit using two monoclonal antibodies (PE10 and PC6) against human surfactant protein A (SP-A) has been shown to be useful for the determination of SP-A in human amniotic fluid. Using this immunoassay kit, we detected a substance in human serum which reacts to the monoclonal antibodies. Immunoblot analysis revealed that the substance in human serum was an 80 kDa protein which shares the same epitope with SP-A. The 80 kDa protein was designated as PE10-reactive substance ("PE10-RS"). The PE10-RS levels were measured in the sera of patients with lung epidermoid carcinoma, lung adenocarcinoma, stomach carcinoma, sarcoidosis, idiopathic interstitial pneumonia (IIP), pulmonary alveolar proteinosis (PAP) and healthy volunteers. The content of PE10-RS in sera was significantly increased in lung adenocarcinoma, HP and PAP, but not in lung epidermoid carcinoma, stomach carcinoma and sarcoidosis in comparison with that in healthy volunteers. In patients with IIP, there was a significant correlation between the PE10-RS level and the serum LDH level. In addition, PE10-RS content in the sera of patients with IIP and PAP was well correlated with the activity and progression of the disease. In conclusion, the determination of PE10-RS levels in sera by the SP-A assay kit seems to be useful for evaluating the progression of lung adenocarcinoma, IIP and PAP.
