The histological change in cerebral venous infarction is characterized by marked brain edema with scattered parenchymal hemorrhages, often conglomerating into a hematoma with mass effect creating problems in neurosurgical operations where the major cerebral venous channel may sometimes have to be sacrified. However, there have been few pathophysiological studies and the specific character of this condition remains uninvestigated, mainly due to the difficulty of obtaining suitable experimental models. In this report an experimental production of a regional venous infarction was attempted in the rabbit by obstructing the cortical vein over the cortex opticus using electrocautery via craniotomy. The venous drainage from the cortex opticus consists of one or two large veins. Obstruction of these veins along the whole length by electrocautery resulted in hemorrhagic infarction within 24 hours in approximately 70% of the animals. The lesion was a wedge-shaped or semicircular-shaped area of infarction consisting of poor staining, vacuolation and ischemic cell changes. There was marked edema in the white matter and many foci of perivascular hemorrhages were widely distributed beyond the border of the infarcted territory. Electron microscopic observation showed enlargement of the spaces around the capillaries with leakage of the erythrocytes, although the tight junction of the endothelium and the number of cytoplasmic vesicles were unchanged. Rupture of the vessel wall was observed in the capillary. The rCBF was measured using a Laser-Doppler flowmeter on the surface of the cortex opticus and showed a decrease within 30 minutes after venous obstruction reaching the value of approximately 80% in 3 hours. The effect of systemic arterial hypertension was studied with animals made hypertensive by the injection of metaraminol. Leakage of injected Evans blue and the incidence and degree of hemorrhage in the area of venous infarction were augmented by the presence of systemic hypertension.
