Circulating immune complexes (CIC) in eleven patients with mixed connective tissue disease (MCTD) were investigated by Clq solid phase radioimmunoassay (RIA) and solid phase anti-C3 RIA in relation to the disease activity. In addition, components consisting of the CIC, in particular, a ribonucleoprotein (RNP) antigen and an anti-RNP antibody were analyzed by the Western blotting method. The following results were obtained: 1. A significant increase of CIC was observed in serum samples from patients with MCTD as compared to those of progressive systemic sclerosis, polymyositis and normal controls. The CIC level in patients with MCTD, however, was far lower than that of systemic lupus erythematosus. 2. The CIC level was correlated with the disease activity in most cases of MCTD. There was a significant inverse correlation between the CIC level and the serum C3 level (p<0.05). There was, however, no correlation between the CIC level and the titer of anti-RNP antibody. 3. Main antigenic determinants of extractable nuclear antigens (ENAs) in rabbit thymus extract (RTE) were analyzed by the Western blotting method. An RNP antigen was identified as 66 K dalton and 40 K dalton, an Sm antigen as 16 K dalton, an SS-A antigen as 85 K dalton and 50 K dalton, and an SS-B antigen as 42 K dalton. 4. Not only an anti-RNP antibody but other undetermined anti-ENA antibodies were detected in serum samples from patients with MCTD. 5. By the Western blotting method, no RNP antigen was detected in the CIC from all MCTD patients tested. On the other hand, substantial amounts of the anti-RNP antibody were detected in the CIC. The titer of anti-RNP antibody in the CIC seemed to be associated rather with the CIC level than with serum level of anti-RNP antibody. These results suggest that the CIC level in patients with MCTD may relfect the disease activity and that the CIC may be mainly composed of the anti-RNP antibody.
