Clinical studies were performed to ascertain the mechanisms of hemolysis and method of prevention against hemolysis due to the administration of fentanyl solution in 53 cardiac patients. The results were as follows: 1. When 100μg/kg of fentanyl solution was injected to various cardiac patients via the peripheral vein for 15min., plasma hemoglobin concentration was significantly increased in a dose-dependent manner. 2. There was a significant increase in plasma hemoglobin concentration in ischemic heart diseases and valvular diseases compared to non-cyanotic congenital heart diseases. 3. Prevention of the hemolysis after the administration of fentanyl solution were investigated: (1) When the dose of 100μg/kg fentanyl solution was administered for 30min. via the peripheral vein, there was no significant decrease in plasma hemoglobin concentration in a group whose injection time was 30min., as compared to the group of 15min.. (2) When fentanyl solution was injected via the central vein, there was a significant decrease in plasma hemoglobin concentration compared to that of the peripheral vein, but a significant increase compared to the control (0μg/kg). (3) When isotonic fentanyl solution containing 10%NaCl was administered via the peripheral vein, there was a significant decrease in plasma hemoglobin concentration in the isotonic fentanyl solution group compared to the hypotonic fentanyl solution group. 4. 10%NaCl added to the hypotonic fentanyl solution showed no adverse effects on hemodynamics and on the depth of anesthesia. In conclusion: The addition of 10%NaCl was the most simple and effective way to alleviate hemolytic effect of the fentanyl solution.
