E. coli endotoxin administered to rabbits at the beginning and end of a 24 hour period provoked the Shwartzman reaction in selected liver lobes. The reaction was compared with a single intravenous infusion of endotoxin. The effect of administering antithrombin-III and heparin on the univisceral Shwartzman reaction of the liver (hepatic Shwartzman reaction) was examined. The following results were obtained. (1) Four out of 5 lobes of the liver in one rabitt were provoked the Shwartzman reaction. Namely, the lobe-selective Shwartzman reaction of the liver was provoked. (2) Extensive hepatic cell necrosis was found in the lobes provoked the hepatic Shwartzman reaction. Only slight necrosis was observed in the lobes which were not preparatively injected. The change of the kidney was hardly recognized in the three groups. (3) The hepatic blood flow decreased dramatically in every group and in every lobe, but in the hepatic Shwartzman reaction provoked lobes, only a slight decrease was shown. (4) In liver function tests, the single intravenous infusion of endotoxin group and the hepatic Shwartzman reaction group recorded a slight increase of GOT and GPT activities. There was a remarkable increase of β-glucuronidase activity. (5) Plasma endotoxin concentration was markedly elevated an hour after provocation. In three hours, plasma endotoxin concentration was slightly diminished, but thereafter elevated again. (6) Antithrombin-III and heparin treatments improved liver function tests, and had an effect on some rabitts to prevent hepatic necrosis. The above results suggested that the lobe-selective Shwartzman reaction of the liver could be provoked. The hepatic Shwartzman reaction had a extensive necrosis, but prevented hepatic blood flow from decreasing. Hence, there would be little influence on blood coagulation abnormalities in occuring necrosis.
