気管支鏡検査は重大事故の発生率という観点からは安全な検査であるといえる。しかし、発生率が低くても検査中に合併症が発生すると重篤な傷害に進展する危険性が高い。気管支鏡検査時の合併症のうち頻度が高いものは、大量出血、気胸、呼吸不全、喘息発作、心筋梗塞、不整脈、リドカイン中毒、炎症、気道閉塞の悪化などと報告されている。気管支鏡検査での意識に上がるリスクを軽減するには、インシデントレポーティング制度よりも、検査中に気がついたことが不安全状態かどうかを判断して、不安全状態であればすぐに改善することがよいと思われる。また、合併症が発生した場合を想定してシミュレーションを繰り返し行い、フェイルセーフが十分であるか絶えず検討することが望ましい。発生頻度は極めて少ないが、重大事故につながる事例とその対策については、投稿論文や学会発表などを通じて検査グループや施設内での知識として共有する必要がある。
