Interest in granulocyte transfusion therapy has been rekindled by both modern leuka-pheresis techniques and the use of Granulocyte Colony-Stimulating Factor(G-CSF)to mobilize polymorphonuclear leukocytes in normal donors. But the risks to the donor need to be carefully considered. Reports of spontaneous splenic rupture occurred after stem-cell mobilization, bone pain, and arthralgia in healthy donors pretreated with G-CSF are of concern. These observations ought to be temper the enthusiasm in the clinical use of granulocyte transfusion therapy. Potential risks of donor mobilization regimens should be considered in granulocyte transfusion programs. Although serious long term effects of short-term administration are unlikely, to establish this convincingly it is important to carefully follow adequate of these individuals. We should emphasize that the short and long-term safety of the practice of treating normal donors with G-CSF is an important concern for the donor-based granulocyte transfusion therapy.
