A recent advance of safe and appropriate practices of autologous blood transfusion in Japan was reviewed briefly. A preoperative autologous blood donation program is a more popular method of autologous blood donation in Japan than a normovolemic hemodilution system or a blood cell salvage system. In 2006, a nationwide study by questionaire estimated that 7.6% of all RBC transfusions could be preoperatively donated by and transfused to patients undergoing elective surgery. The practices of preoperative autologous blood donation in the hospitals have been done according to the guidelines determined by the Ministry of Health and Welfare or the standard method recommended by the Japanese Society of Autologous Blood Transfusion. Moreover, clinical path procedures for preoperative autologous blood donation have been widely utilized recently to provide a safer and more appropriate way. Additionally, a new system for certificated nurses certified in autologous blood transfusion was established by the Japanese Society of Autologous Blood Transfusion in 2009, and the 223 nurses certified by 2011 are expected to develop safer and more appropriate practices of autologous blood transfusion.
