Plasmapheresis Combined with Extracorporeal Ultrafiltration Method : A Case Report of a Patient with Diabetic Nephropathy Complicated with Atherosclerosis Obliterans

A 66-year-old male presented with nephrotic syndrome (NS) 1 year after the discontinuation of diabetes treatment. The patient was treated using double filtration plasmapheresis combined with extracorporeal ultrafiltration method. He remained asymptomatic after the treatment. However, at 6 months after the discontinuation of the treatment, he developed recurrent NS complicated with atherosclerosis obliterans. We consequently administered a combined treatment with low-density lipoprotein apheresis and extracorporeal ultrafiltration method. The patient became symptom free, and his hyperlipidemia improved. In the present case, we found that the combined therapies were useful for the ameliorations of NS and atherosclerosis obliterans. We also determined the effects of apheresis columns and found that there were differences in the binding affinities among LDL-cholesterol, fibrinogen, and other lipids.
