僧帽弁、三尖弁同時置換症例の血行動態に関する研究 : EchocardiographyとImpedance Cardiographyによる観察を中心として

Echocardiographic study was done in 13 cases who underwent a simultanous mitral and tricuspid valve replacement. In all cases, the Starr-Edwards disc valve was used, size 3M for the mitral, and size 4M for the tricuspid valve. For the mitral and tricuspid valve prosthesis before and after exertion, the following data on UCG, together with PCG, ECG and Impedance Cardiography, were gotten; the movement ; the length of the opened period ; Ω-closing time (the length of time between the Ω wave on ECG and the instant of closing) ; II-opening time (the length of time between the 2nd sound on PCG and the instant of opening) ; and the velocity of the opening and closing of the mitral and tricuspid valve prosthesis. The following results were obtained. 1) The movement of the mitral and tricuspid disc valve prosthesis was good. In 13 cases, the valve dysfunction was not discoverd. 2) The period that the mitral prosthesis was open was always longer than that of the tricuspid prosthesis, and the relation was unchanged at rest and also after exertion. 3) In tricuspid prosthesis, the Ω-closing time was longer and the II-opening time was shorter than that of the mitral prosthesis. This suggests that the state of the functional stenosis at the tricuspid prosthesis was more remarkable than that of the mitral prosthesis.
