1) I developed an original new method of nonsuture microvascular anastomosis using magnet rings and cogwheel-shaped hollow metal instruments with 6 spurs or oval hollow instruments, With this method, both end-to-end and end-to-side anastomosis are possible. 2) End-to-end anastomosis was possible for small vessels with outer diameter up to 1 mm with average patency rate of 90%, requiring 8.0 minutes on average. In the case of end-to-side anastomosis, patency rate was 84%, requiring 8.4 minutes on average. 3) In end-to-end anastomosis, electromagnetic flowmeter measurement showed that there was no difference of blood flow and blood flow patterns between controlled and operated vessels, even 40 days after anastomosis. In end-to-side anastomosis, blood flow at donor vessel and the total blood flow at recipient vessel were same even after 40 days in the case of artery-to-artery anastomosis, and also after 60 minutes in the case of artery-to-vein anastomosis. A change of blood flow pattern was not recognized. 4) Histological examination 20 days after anastomosis revealed a continuity of the media and the intima. Even after 180 days, no marked abnormal findings were recognized. By scanning electron microscope study, disruption and flattening of longitudinal endothelial folds on the inner surface of the vessels were not observed. Histological examinations showed that magnetic power did not have jmy effect on blood vessel walls and surrounding tissues. 5) From the above reasons, this method of nonsuture microvascular anastomosis proved to be an excellent one with a possibility of clinical application to bypass surgery for occlusive cerebrovascular disease in neurosurgical field.
