The relationship between ocular sarcoidosis and adenosine deaminase (ADA) isozyme activity was investigated by using the AD-test. Thirty-four patients (Pts) with sarcoidosis and 52 healthy controls were examined. t-ADA and ADA-2 activity in the sarcoidosis group was significantly higher than in the control group. Seventeen of the 34 sarcoidosis Pts, had high t-ADA and ADA-2 activity (positive-Pts). The 7 Pts in the sarcoidosis group treated with Predonine^[○!R] had lower activity of all ADA isozymes than the 27 untreated Pts. The 27 Pts were classified according to the severity of their ocular disease into: acute, stable and inactive. t-ADA activity in the acute group was significantly higher than in the stable group, and the percentage of the positive-Pts was 90%. The percentages of acute, stable and inactive Pts positive for angiotensinconverting-enzyme (ACE) were 70, 72.9 and 33.3%, respectively. Thus, ADA isozymes were better correlated with ocular disease than ACE. Only ADA isozymes were significantly active in Behget's and Harada's disease. This suggests that t-ADA and ADA-2 activity is useful for detecting acute disease alone, however, ACE activity is a highly sensitive parameter for both acute and stable ocular sarcoidosis.
