To study the viability of the warm ischemic canine pancreas, ^<31>P-NMR spectroscopy was used to measure β-ATP under the following conditions: (1) cardiac arrest or cessation of blood flow, and (2) cessation and restoration of blood flow. An intravenous glucose tolerance test was also done after restoration of blood flow. The level of β-ATP decreased gradually and became undetectable within 40 minutes of cardiac arrest or cessation of blood flow. The level of β-ATP returned toward normal after restoration of blood flow, and the rate of return was significantly higher in the 1-hour warm ischemia group. During the glucose tolerance test, the blood glucose levels did not change significantly. But the immunoreactive insulin levels in the control and the 1-hour warm ischemia group were significantly higher than that in the 2 and 3-hour warm ischemia group. These data indicate that β-ATP is retained by the warm ischemic pancreas for 40 minutes, and that the rate of recovery decreases as the duration of warm ischemia is extended. Because of the effects of operative stress or anesthesia, intravenous glucose tolerance testing soon after warm ischemia may not be an appropriate way to evaluate pancreatic exocrine function.
