Eighty two eyes of forty five patients with Behcet's disease were enrolled in this study to identify unknown factors associated with the structures adjacent to the ciliary body in vivo. High punctate reflection using ultrasound biomicroscopy in the anterior chamber suggesting inflammatory cells was observed in 42.9% of the eyes unaffected by ocular attacks for more than 1 year. Eyes with reflection had frequent severe attacks, and the visual acuity, fundus score, and aqueous flare intensity of these eyes were worse than the eyes that did not exhibit reflection (p=0.0004, p=0.0020, p=0.0087). The pars plana was thicker in Behcet's disease (330.5 ± 68.9: mean ± SD) than in the normal controls (268.9 ± 33.5) (p< 0.0001), and it was significantly thicker in eyes within 2 weeks after an ocular attack than in eyes more than 2 weeks after an ocular attack (p = 0.0097). A significant positive correlation was found between pars plana thickness and mean fundus score for over the preceding year (r=0.753, p < 0.0001). The pars plana of eyes with punctate reflection in the anterior vitreous was thicker than in eyes without punctate reflection and in normal control eyes (p=0.0013, p<0.0001). Examining the structures adjacent to the ciliary body by ultrasound biomicroscopy would be valuable in detecting clinical severity and assessing the efficacy of medication and deciding on drug therapy.
