蕁麻疹患者における血漿トリプターゼ値の臨床的意義に関する研究

Plasma tryptase and histamine concentrations were assessed in 61 patients with chronic urticaria, 17 patients with acute urticaria, 15 patients with atopic dermatitis, and 26 healthy controls. Plasma tryptase was measured by fluoroenzymeimmunoassay using UniCAP tryptase and histamine concentration was measured by radioimmunoassay. Plasma tryptase levels during the active periods were significantly higher in patients with chronic urticaria (p<0.05) and acute urticaria (p< 0.0005) than the healthy controls while the difference in histamine levels was statistically insignificant. The tryptase levels during the inactive or well-controlled periods were not significantly increased in patients with chronis urticaria, and acute urticaria when compared with the healthy controls. The result suggests that plasma tryptase level might be a more valuable parameter for activation of plasma cells than the histamine level in urticaria. The relationships between the tryptase level and three clinical parameters, range, degree of eruption, and degree of pruritus, in patients with chronic urticaria were unclear.
