CLINICAL PATHOPHYSIOLOGICAL STUDY ON GASTRIC SECRETORY FUNCTION THROUGH RELATIONSHIP BETWEEN MUCOSAL PEPSINOGEN AND PEPSIN OUTPUT IN THE HUMAN STOMACH

The purpose of the present study is to elucidate the secretory mechanisms through mucosal pepsinogen of the stomach and pepsin output in the gastric secretion. The pepsinogen levels of gastric mucosa and pepsin output in 19 duodenal ulcer (active stage) patients were significantly higher than those (mucosal pepsinogen of body: 2.12±0.96 μg tyr./mg prot./10min, BPO: 33.28±31.67 mg tyr./h, SPO: 113.82±56.19 mg tyr./h) in 20 normal subjects, and they lowered with the healing of duodenal ulcer. Pepsine output and mucosal pepsinogen levels were higher in gastric ulcer patients than in normal subjects. The secretory function of pepsin were depressed with the recovery of the gastric ulcer. BPO were fairly decreased in gsatric cancer patients (both early and advanced) either before or after classifying the fundic gland area, although no difference from normal subjects was seen regarding the mucosal pepsinogen. It was suggested that hypersecretion of gastric pepsin in duodenal ulcer (active stage) patients was due to impairment in the homeostatic mechanisms caused by either local neurogenic stimulation or hypergastrin. According to histological characteristics of cancer, uninvaded mucosal pepsinogen levels in advanced gastric cancer patients were slightly higher in differentiated type than in undifferentiated type, but both BPO and SPO in differentiated cancer group were lower than in undifferentiated.
