• Gastric biopsy and brush cytology under direct vision were evaluated. The diagnostic accuracy of biopsy was 94.1%, that of cytology was 88.2%, and a combined use of biopsy and brush cytology gave a higher diagnostic accuracy of 97.1%. Gastric cytology has become supplementary to biopsy because of the excellent diagnostic accuracy of biopsy. The clinical value of gastric brush cytology, however, has to be emphasized in the cases where biopsy forceps cannot reach the tumor itself due to the stenosis or location of the lesion, and where enough material cannot be taken from the lesion because it is a very tiny carcinoma or carcinoma cells present mainly beyond the submucosa. Some benign atypical cells had to be distinguished from adenocarcinoma cells.
