Antibody-dependent cell-mediated cytotoxicity (ADCC) was determined by the microplate method in 44 patients with sarcoidosis, and spontaneous lymphocyte-mediated cytotoxicity (SLMC) by ^<51>Cr release assay in 33 patients. The results were as follows: 1) In the control group , ADCC was shown to be independent of the donors' age and sex, whereas SLMC was only independent of the donors' age. SMLC of the male in the control group was higher than that of the female, but statistically not significant. 2) ADCC as well as SLMC in these patients was significantly higher than in the control group, and higher in active cases than in inactive cases. 3) Both ADCC and SLMC increased in patients with bilateral hilar lymphadenopathy and/or pulmonary infiltraction as compared to those in patients with no abnormal findings on chest roentgenography. 4) ADCC, moreover, had a significant correlation with serum angiotensin-converting enzyme and lysozyme activities, probable indicators of the disease activity of sarcoidosis.
