Data on ninety-six cases of failures and associated problems which occurred to machinery in the engine room, provided by shipowners in Japan in response to a questionnaire, was analyzed from the perspective of the influence of human factors. The results of the analysis show that so-called 'hard' errors (i.e. errors directly attributable to machinery problems) and 'soft' (i.e. human-caused) errors occurred at a ratio of approximately 1:1. Additionally, a greater number of perceived errors occurred while the vessel was underway, although a larger number of errors due to misoperation and misunderstanding occurred together with perceived errors while the ship was in a non-navigation state in cases of failure or trouble which were due to human errors. Further data analysis was carried out using damage data from periodical and occasional classification surveys in order to verify these results.
