In order to determine the clinical and pathological significance of C3 deposition on the arteriolar walls of the kidney, renal biopsies from 236 children with nephropathy were examined prospectively by immunofluorescence microscopy. Vascular C3 deposition was found in 97 cases (41%). Although it occurred with a variety of clinical disorders and histological diagnoses, it was associated particularly with hematuria, slight proteinuria and minor glomerular alternations. Autopsy specimens from children without renal diseases showed no vascular C3 deposition. These observations suggested that C3 deposition might damage the arteriolar walls and cause hematuria in these children.
