大気汚染に関する医学研究 : 質問票を中心とした集団検診による慢性呼吸器症状の摘出法

This study aimed to know that the prevalence of nonspecific respiratory symptoms of persons workig in an area checked by mass medical examination; such as (1) British Medical Research Council's short questionnare (BMRC), (2) pulmonary function test using vitalor and (3) X-ray examination for the chest, could distinctly reveal the degree of air pollution in this area. In other words, the major object of this study was whether the numbers of persons having symptoms screeninged by those techniques could be correlated with the degree of air pollution in the area or not. The study was performed on 643 workers engaged in three factories in Chiba manufacturing district, workers could be classified into four groups accordance with different characteristics of air pollution in the areas each group belonging to. Characteristics of air pollution in four areas were as follows; A was highly polluted with coal and metal dust, B was polluted with the same substances as A but less polluted than A, C was the least among A, B and C group, D was slightly polluted with dust and the other kinds of particles but certain kinds of gaseous substances originated from some organic solvents were found in its atmosphere. Data obtained were as follows; (1) Prevalence of cough (BMRC; 1/3+5) could be illustrated in the order of their size as bellow; A>D>B>C (2) Prevalence of phlegm (BMRC ; 6/8+10) could be illustrated in the order or their size as bellow; D>A>B>C (3) Prevalence of shortness of breath (BMRC 14a+b) could be illustrated in the order of their size as bellow; D>A>B>C In general, from the point of view of summing up of cough, phlegm and shortness of breath the four groups might be illustrated in the order of nonspecific respiratory symptoms as bellow ; A>D>B>C (4) The numbers of abnormalities in persons checked by percent vital capacity (i.e. FEV_<1.0>/FVC%) were illustrated in the order of their size as bellow; A>B>C>D (5) The numbers of abnormalities in persons checked by the vital capacity (i.e. %VC, FEV_<1.0>/FVC% or the both) were illustrated in the order of their size as bellow; A>B>C>D (6) The numbers of persons with abnormal findings in the chest checked by X-ray examination were illustrated in the order of their size as bellow; A>B>C>D Conclusively it will be able to say that the results obtained from the persons working in the dusty areas by so called mass medical examination including BMRC's questionnare, are reliably correlated with the degree of air pollution with dust and the other kinds of small particles. While, when cartain kinds of specific gaseous air pollution come into the question, the prevalence of cough, phlegm and shortness of breath obtained by BMRC's questionnare ; the abnormalities checked by the pulmonary function test ; the abnormal findings in the chest checked by X-ray examination are usually differd from each others, when they are compaired in the order of their size.
