Blood abnormalities in gastric insufficiency were studied from hematological viewpoint particularly on vitamin B_<12> with clinical method in 69 cases of atrophic gastritis, 47 cases of gastric polyp, 51 cases of gastric cancer, 73 cases of gastrectomy, and 9 cases of pernicious anemia. Gastric lesions in pernicious anemia were studied simultaneously. 1) Anemia noted in 30% of cases of atrophic gastritis and gastric polyp was mostly mild. In both diseases, low values of serum iron level were noted in some cases. In atrophic gastritis, serum vitamin B_<12> concentration was at the lower limit of normal or decreased in many cases. Macrocytic tendencies in some cases, low serum vitamin B_<12> values, and high serum iron level in some cases suggested vitamin B_<12> deficiency. In gastric polyp, macrocytic tendencies, high serum iron level, high serum vitamin B_<12> level, and decrease in granulocytes and lymphocytosis in the bone marrow in some cases suggested hypoactivity of hematopoietic function, although low serum vitamin B_<12> level was not encountered. These findings are apparently related with the development of blood abnormalities in these two diseases. 2) In gastric cancer, anemia was seen in one-half of cases and mostly hypochromic. Low serum iron level was also seen in one-half of cases. Abnormalities in iron metabolism apparently are the major participants. Serum vitamin B_<12> was slightly elevated in many cases. 3) In gastrectomized subjects, cases of anemia increased with the course of time following the operation. A tendency of decrease in serum vitamin B_<12> was seen especially in totally gastrectomized subjects. Disturbance in the absorption of vitamin B_<12> was seen even in partially gastrectomized subjects. Possibility of the development of iron deficiency anemia was noted any time through the course of the illness. 4) The presence or absence of inflammatory changes in the atrophy of gastric mucosa in pernicious anemia has been discussed. Inflammatory changes were seen in some cases, and the gastric mucosa was unchanged after treatment. 5) When a large amount of vitamin B_<12> was administered orally, absorption took place even without intrinsic factor. 6) Vitamin B_<12> values were determined in about 240 cases of various diseases in the field of internal medicine.
