The electrokymography was made in 101 subjects including 28 subjects of normal health, 30 cases of hypertension, 19 cases of ischemic heart disease, 14 cases of mitral valvular disease, 10 cases of bronchial asthma and emphysema. The electrokymographic curves of the left atrium, the right hilus and the lung field were investigated with special reference to the left atrial function. 1-i. The morphological classification was made in the left atrial curves of normal health. Furthermore relative amplitude (especially of the presystolic and diastolic descent) and the time phase (especially the atrial electromechanical latent time, atrial systolic duration, isometolic diastolic period, and the rapid filling period) were measured. As mentioned hitherto, each measured value was pretty variable, so that it was interpreted cautiously. The increase in the rate of the presystolic descent and the decrease in the rate of diastolic descent tended to be seen in the senile group, but there could not be observed any difference between the juvenile and senile group in the time relation. 1-ii. The atrial curves were analysed in arrhythmia, especially atrial fibrillation. 1-iii. Observation on the left atrial curves in cases of mitral valvular disease reconfirmed the previously emphasized finding. 1-iv. The left atrial curves in cases of bronchial asthma and emphysem were compared with those in normal health. There could not be found any clear-cut difference between these two groups. 1-v. The left atrial curves were studied in hypertension and ischemic heart disease. As compared with the normal group, the rate of the presystolic descent tended to be augumented in these groups. On the contrary, the rate of the diastolic descent tended to be reduced. Respecting the time relation, the isometolic diastolic period was prolonged, but no difference was noted in the other measured values. 2. The eky curves of the right hilus and the lung field were examined in mitral valvular disease, hypertension, and ischemic heart disease.Different from the normal group, the diphasic curves with the presystolic positive waves were seen in these diseases. This presystolic positive wave was assumed to be of atrial origin produced by the augumented change of venous blood flow. This pathologic finding occupied a high percentage in the groups above mentioned. In cases of hypertension, this high percentage was related to the degree of severity. On the other hand, a relationship was obtained between this high percentage and pathologic findings of p wave in ecg (widening of p wave, or p-sinistrocardiale) For the pathogenesis of these pathologic findings in hypertension and ischemic heart disease, the rise in the endo-diastolic pressure of the left ventricle was suggested.
