How to judge the prognosis of spinal caries through X-ray examination? On this theme, hitherto, it has been reported that the diagnosis through the radiographic appearances is not necessarily difficult in the following way; first we classify the radiographic appearances into some types, complete block type, partial blook type, et al, and when the condition of the disease settles, we decide to which type it belongs and then investigate the relation with the classification of the radiographical appearances at the beginning of the disease. In this way, it has been said, we can catch the characteristics of the form of the spread and the prognosis of each type of disease. But, I, the auther of this report, thought that in the judgement of the prognosis of spinal caries, the probability of recovery should be taken a serious view, irrespective of the radiographic appearances, and so I classified the radiographic appearances into two types, a type which has a tendency to recovery and a type which has not such a tendency. The former includes complete block type, partial block type, and et al. From this point of view, since July of 1947, I have systematically investigated 230 cases of the radiographic appearances of the spinal caries that I have met with in Chiba National Hospital. More over, I injected into marmot, knee joint the following three: (1) tubercle bacillus. (2) tubercle pus (negative bacillus culture) (3) mixture of tubercle bacillus and tubercle pus. And as to these three groups I compared and examined the pathological tissue. As the result, the first group that was injected the tubercle bacillus was characterized with the immediate appearances of transsudation and in 6-8 weeks, it came to its zenith. But since then, there appeared the tendency to proliferation and became calm. In the second group which was injected the tubercle pus, at first there can be observed the prolifaration of the epithelium of synovialmembran, inflammatory cellinfiltration, and lixivium, but ten weeks after there cannot be observed any abnormal conditions. On the contrary, in the third group which was injected the mixture of bacillus and pus, there can be early observed the proliferous change and the increassation of soft tissue in the joint, cell-proliferation, new born blood vessel, and the formal tuberculous node are accompanied and distinctive curable tendency can be observed. But 7-9 weeks after, there apeared transsudation and the tendency of the malacy of the ill part. After all, the existence of tubercle pus stimulates at first the prolifer ous activity in the ill part and after that as the accumlation extends over a long space of time, there can be observed the accompany of transsudation and becomes to prevent the recovery of caries.
