I observed serially, a reaction of blood transfusion in blood disease and in various anemic condition on the base of alteration of blood cell and platelet, and then discussed a hemostatic effect of blood transfusion. Results: 1. Red blood cell count Blood transfusion from 100 to 200 cc (whithin 500 cc) does not produce decrease or increase of red blood cell count in spite of any anemic condition of receptor. There is a special influence to an alteration of leucocyte. The influence comes from increase or decrease of leucocyte of a receptor or various diseases showed highly abnormality of hematology. 2. Leucocyte (1) White blood cell count White blood cell count easily alters by blood transfusion. Count decreases within 60 minutes and increases later on 2 hours after the tranfusion. (2) Picture of white blood cell I observed an indefinite type of reaction of a blood transfusion exclussive of 4 types of H. Wigand's classification, namely stress, allergy, biphase and no reaction. There is no relation between stress type of blood picture and clinical symptom of stress. In case of allergic side effect show an allergic reaction of blood picture. The allergic type does not always show an allergic symptom. In case of aplastic anemia, type of indefinite reaction notes numerously, and there is no reaction type in many cases of acute leucemia. (3) Main group of cell controlling alteration of white blood cell Matured granulocyte and lymphocyte plays a leading role to alter the leucocyte. In case of acute and chronic leucemia, it observes a special finding to mobilize the young cell into the blood stream. (4) Intramedular transfusion and a quantity of blood There is no special alteration by an intramedular transfusion of 100 cc or an intravenous transfusion, of blood from 50 to 500 cc. 3. Platelet count The platelet increases within 2 hours and decrease later on 3 hours after the transfusion. Then the platelet count gradually increase as same as that before the transfusion later on 5 hours after the transfusion. There is no special type of alteration. In the group of highly thrombopenic disease also shows an increase of platelet cout. But an increasing rate of platelet in case of aplastic anemia, acute leucemia or idiopathic thrombopenic purpura is less than that of none thrombopenic disease. There is much increase of platelet in case of side effect of blood transfusion. As far as blood quantity, there is no influence up 50 to 500 cc of blood transfusion. An intramedular transfusion shows no evidence of special alteration. 4. Hemostatic effect and platelet count There is a marked effect of hemostage from immediate later to 24 hours after the transfusion. There is, however, no evidence of definite relation between the hemostatic effect and the alteration of platelet. The intramedular transfusion does not give any better hemostatic effect than intravenous transfusion. 5. Mobilization of leucocyte by reaction of transfusion or by adrenalin There is a certain relation between a mobilization of leucocyte by reaction of transfusion and that by adrenalin.
