所謂"肺結核腫"の臨床とその病理学的研究 : 特にその手術適応について

Studies were made of the so-called "tuberculoma" in its clinical and pathologic aspects. Clinical and roentgenographic examinations were carried out in 226 patients with pulmonary tuberculosis, in whom there had been evidence of tuberculomas (360 foci all told), hospitalized at our sanatorium during the period from 1949 to 1957. Histopathologic studies were carried out on 73 tuberculomas found in the specimens of the lung resected from 58 patients. The results are summaried as follows : (1) The fact that most of the tuberculomas found in the specimens of the lung studied have a structure resembling rings or are of fused type seems to indicate that they are the result of complicated localized process. (2) When roentgenograms of the chest of patients with masses thought to represent tuberculomas revealed evidence of (1) the process of cavitation being taking place, (2) the presence of a focus over 2cm in diameter or (3) open bronchial communication with the hilum or bronchial involvement, the roentgenographic appearance of the tuberculous lesion was unstable. There was histopathologic evidence in support of this observation. It seems that this observation merits further clinical consideration and that it may help in deciding whether surgical extirpation is desirable.
