Statistical figures show that not only among cancer patients treated in this clinic but also among all the cancer patients reported in Japan cancer of the rectum occupies third place in the frequency of occurrence after cancer of the stomach and cancer of the esophagus. It would not bs justified, therefore, to make light of the diagnosis and treatment of cancer of the rectum in the war against cancer. The author made statistical observations of the patients with cancer of the rectum who had been treated in this clinic during the period of 11 years and a half from January, 1946 to June, 1957. It was ievealed that the statistical figures on the frequency of occurrence, age distribution, hereditary influence and the past medical history of these patients were more or less the same as those that had been reported by other investigators, both at home and abroad. Abdominosacral proctectomy is a surgical procedure devised and performed successfully in this clinic on patients with cancer of the rectum. The results following the treatment by this method are considered to be satisfactory from the standpoint of physiology. The author compared the end-results of the patients treated by this method with those operated on by other procedures. The study indicated that the patients treated by this method had larger chances of cure with lower rate of death from the hazards associated with any operation. The study also revealed that the status of the patients' health after the operation was governed by: (1) the location of the tumors and their site on the wall, (2) the presence or absence of metastasizing foci in the lymph nodes, (3) the period before operation after some of the early symptoms had been detected (evidence that early diagnosis and treatment are the best possible guard against cancer) and (4) the presence or absence of stricture. Proctotoreusis in the perineum was physiologically most satisfactory. Early recovery of anal function can be secured if the sphincter has not been removed. Curves representing the movements of the anus in function tests testified to this.
