The author performed roentgenographic studies of duodenal functions, as well as clinical tests for pancreatic functions, in 50 patients with ulcers of the stomach and duodenum, stomach cancer and chronic gastritis who had undergone gastrectomy. The study, which was conducted in order to correlate the functions of the duodenum with those of the pancreas after gastrectomy, disclosed the following findings: (1) There was marked improvement in pancreatic functions three months after gastrectomy. (2) Return to normal functioning of the roentgenological findings was delayed in patients operated on by the procedure B II as compared with those operated on by the procedure B I. (3) It may be said that too quick evacuation of stomach contents is one of the important factors responsible for impairment of pancreatic functions. Duodenal delay, which occurs after gastrectomy, may counterbalance to a certain extent the defect of gastric function. It is not necessarily a factor in the impairment of pancreatic function. (4) It will be seen from these observations that the procedure B I is preferable to any other methods. But there may be cases where cutting out a large portion or the use of the procedure B II is required. In these instances, excision and anastomosis should be carried out with the best possible care so that too quick evacuation of stomach contents may not occur.
