The author conducted statistical investigations of the follow-up results as well as both the preoperative and postoperative conditions of 280 cases with cancer of the stomach who underwent Nakayama's gastrectomy. The resected specimens from 90 patients with this disease were also subjected to pathological study. These results turned out to be as follows: 1) Both postoperative and further follow-up results of those patients who underwent Nakayama's Gastrectomy have been excellent in general except only some cases in which the general condition was somewhat aggravated. 2) Neither serious complications nor sequelae attributable solely to this operative method itself have ever been noted. 3) This method seldom failed to remove cancerous tissues at the resected stump of the stomach. Albeit this fact, even in the case with gastric cancer displaying relatively good nature, it is always far wiser that gastrectomy is done at least 5-6 cm away from the tumor, if it involves the cardiac segment, and that resection is carried out at least 3 cm away from the lesion, if it invades the pyloric portion. Therefore, in the case with carcinoma tending to infiltrate the abjacent structures, meticulous attention should be paid to decision of the scope of resection. If care should be taken to this matter, much better results would be expected. 4) If metastases of cancer to the lymph nodes have taken place, the involved lymph nodes should be thoroughly removed with the aim of improving the follow-up results as much as possible.
