肺癌脳転移切除例に対する開頭術中照射の意義

Between 1979 and 1990, 28 patients with brain metastases from lung cancer were treated with surgical resection and radiation. The overall survival rate was 13%. Of these 28 patients, 7 received intra-operative radiation (IOR) followed by whole brain radiation, and 5 received IOR alone; the 5-year survival rates were 16.7% and 33.3%, respectively, suggesting that whole-brain radiation is not always necessary. Furthermore, mental and neurological deterioration due to whole-brain radiation were observed frequently in the former group, while none of the latter group sufferred from these complications. These results indicate that surgical removal combined with IOR alone may improve survival and avoid mental deterioration which occurs frequently in patients receiving whole-brain radiation.
