The Public Long-Term Care Insurance System, implemented in April 2000, was developedby totally reorganizing the then current welfare system for the elderly in combination withmedical services for those requiring care. The primary aim was to provide a large part ofconventional welfare services for the elderly through the social insurance service. The structure of long-term care service fees was dramatically changed, and being laterrevised twice in order to accommodate physically or mentally serious cases. Because of these revisions in service fees, many special nursing homes for the elderly havecaused two main problems to emerge:1. Due to their decline in earnings, many facilities have become more inclined towards providing care to serious cases because of the higher fees available, rather than normal cases where fees are very low.2. Decline in their ability to function as a safety net for those suffering from abuse, neglect, or other social environmental reasons. Instead, many facilities have a higher preference for those certified as eligible for long-term care. Countermeasures to address these problems are necessary. Temporarily, a balanced chargesystem should be adopted in order to stabilize facilities' operations, as well as futurederegulation of medical treatments prohibited within facilities. The overall restructure shouldbe made to focus on providing care not just to those certified eligible for long-term care, butalso to those suffering from social environmental circumstances. Both public and governmental opinion has been very critical of nursing home exstence, butsome facilities will have to engage in a providing care to all the elderly in need and theirfamilies, whereby 'peace of mind' and 'human dignity' should prevail.
