Evaluation of filtering bleb imaging using a new optical coherence tomography

Objective : To evaluate filtering blebs present after trabeculectomy (LET) with anterior segment optical coherence tomography (OCT) using a new technique - optical frequency domain reflectometry - OCT (OFDR-OCT). Materials and Methods : Thirty-one eyes of 26 patients were evaluated. All patients had been treated by LET with mitomycin C and displayed good postoperative intraocular pressure (IOP) control. The inside of the filtering bleb was evaluated in detail using OFDR-OCT, and each bleb was classified into 1 of 3 types, according to the findings. IOP and reduction in IOP from the preoperative value were compared among the 3 types. Results : The OFDR-OCT could be readily and rapidly performed, which allowed detailed evaluation of the inside of the filtering blebs, such as bleeding, adhesion, and fibrosis on tomographic images. The mean IOP or the reduction in IOP from the preoperative value did not differ among the 3 bleb types, showing no association with filtering bleb findings observed by the OFDR-OCT. Conclusion : OFDR-OCT provides detailed information on the inside of the filtering bleb. The 3-dimensional evaluation of the entire filtering bleb by this method may be useful in deciding the treatment procedure at the time of an increase in IOP.
