Since 1989, clinical trials of gene therapy have been launched and almost 1300 protocols, two thirds of them for cancer, have been accepted officially and performed at the worldwide level. In Japan, 21 protocols have been accepted officially and 14 of them target cancer. Among cancer gene therapy approaches, immune gene therapy is most popular. It includes cytokine genes of IL-2, IFN-γ, GM-CSF and cancer-specific genes of MUC 1, and HPV-16. A recent report showed the MART-1-T cell receptor gene-transduced tumor-infiltrated lymphocytes were effective for metastatic melanoma patients. Although remarkable clinical effects were reported using several immune gene therapy protocols, more development is definitely required. Recent innovative preconditioning by lympho-depletion before adoptive immunotherapy and the combinatory administration of antibody for cytotoxic-T-lymphocyte-associated antigen 4 might open a new era for immune gene therapy if their most appropriate administration method is established. Further intensive clinical trials as well as basic studies are required to establish safer and more effective immune gene therapy for advanced cancer patients.
