The purpose of this paper is to investigate the characteristics of lipoprotein metabolism, with special reference to the relationship between HDL-cholesterol and diabetic microangiopathy, in which has been studied in 99 healthy subjects and 240 diabetics. The following results were obtained. 1) Plasma HDL-cholesterol concentrations of female healthy subjects and diabetics were higher than male healthy subjects and diabetics. There, however, was no difference between those of healthy subjects and diabetics. 2) Among the diabetics with the treatment of oral hypoglycemic agents, patients with vascular complications showed lower HDL-cholesterol levels compared with patients without vascular complications. HDL-cholesterol levels, however, was showed no difference in those patients with and without vascular complications if compared the patients of same plasma triglyceride ranges. 3) Normolipemic diabetics with retinopathy showed lower plasma HDL-cholesterol levels, HDL particles with less cholesterol content and higher cholesterol distribution in LDL fraction compared with the normolipemic diabetics without retinopathy. 4) Hyperlipemic diabetics with hypertension and/or abnormal ECG showed lower plasma HDLcholesterol levels and higher LDL-cholesterol/HDL-cholesterol ratio compared with the hyperlipemic diabetics without these complications. Hyperlipemic diabetics with retinopathy did not show these findings. From those data it was suggested that decreased HDL-cholesterol level in diabetics with vascular complications treated with oral hypoglycemic agents may be related to the hypertriglyceridemia found in those diabetics and that HDL-cholesterol is one of the factors which is related not only to the macroangiopathy but also to the diabetic microangiopathy.
