We have deveolped a simple determination method of serum proinsulin using insulin degrading enzyme (IDE). The levels of proinsulin, insulin and C-peptide during 50g O-GTT were studied in 41 patients with maturity onset diabetes, 20 patients with borderline type and compared to those in 13 normal subjects. 1) Fasting levels of proinsulin in maturity onset diabetics, borderline type and normal subjects were 0.53±0.03, 0.42±0.02 and 0.24±0.02 ng/ml (mean±S.E.), respectively. 2) A significant correlation between fasting levels of blood glucose and serum proinsulin, but not insulin and C-peptide were observed. 3) Proinsulin levels during 50g O-GTT in maturity onset diabetics and borderline type were significantly higher than those in normal subjects. 4) The responses of proinsulin in maturity onset diabetics were differ from those responses of insulin and C-peptide. 5) The molar ratios of proinsulin/IRI in maturity onset diabetics were significantly increased to compared to those of the other groups. 6) In maturity onset diabetics, diabetics with low proinsulin responses had longer diabetic history, and were less obese than with high proinsulin responses. These results suggest that carbohydrate intolerance and the duration of diabetes are related to the circulating proinsulin, and that there is a clinical difference of proinsulin metabolism between insulin dependent diabetes and non-insulin dependent diabetes. It is indicated that the cause of diabetes and its pathological condition of pancreatic B-cell may be related to the processing and releasing mechanism of proinsulin.
